Santa Clara County has a
special unit of Social Workers dedicated to helping those
families who have already adopted children from our County. We
offer three primary services: Information and Referral,
Financial Assistance, and intensive "Wraparound" Support.
Information and Referral:
Our staff are
available to provide you with referrals to appropriate community
agencies, and to give information on how to obtain
non-identifying information about your adoption, request for
reunion of an adut adoptee, birth parent or sibling, consent for
contact, waivers of confidentiality for siblings, information
about the Adoption Assistance Program in California, and our
Adoption Wraparound services.
Financial
Assistance:
Adoptive parents who have adopted
children from Santa Clara County are usually eligible for
financial assistance, if needed, through the Adoption Assistance
Program (AAP). This assistance can only be granted for
activities or expenses that would have been paid if the child
was still in foster care. Payment levels are reviewed every two
years.
Adoption Wraparound (PACT - Parents Adopting children
Together):
PACT is an innovative service of the County, in collaboration
with Kinship Center's AFTER program in San Jose. Creative, and
highly individualized services are provided to help adoptive
families continue to parent their child or children at home,
rather than placing them in a group home or other out-of-home
